What the evidence says
TMS studies in bipolar depression are small and mixed; some show benefit comparable to unipolar depression, others do not. Safety in bipolar disorder
Treatment-emergent mania has been reported but is uncommon, and a mood stabilizer is continued throughout. Clinics monitor for mood elevation at each visit.
Who is a candidate
Adults with bipolar depression that has not responded to two or more standard treatments, stable on a mood stabilizer, with no seizure disorder or metal in the head.
Coverage and cost
Most commercial policies cover TMS for major depressive disorder and either exclude or treat bipolar depression case by case; some cover it with the same criteria.
