The exclusion
Spravato's trials excluded bipolar disorder, and its label applies only to major depressive disorder. Nearly every insurance policy lists bipolar depression as an exclusion, and clinics screen for it. A bipolar diagnosis anywhere in the chart is one of the most common reasons for a denial. The concern
Ketamine-type drugs can trigger mania or hypomania in people with bipolar disorder, although rates in studies of racemic ketamine with a mood stabilizer have been low.
What to do instead
Ketamine infusions given alongside a mood stabilizer have trial support for bipolar depression and are offered by many clinics. TMS has some evidence and is used off label. ECT is well established for severe bipolar depression and is covered. See ketamine therapy for bipolar depression.
