What we collect

• Coverage checks and the screener. Treatment, state, plan type or insurer, and zip code. These are processed to produce your result and are not linked to you unless you submit the contact form.
• Contact form. Name, email, mobile phone, zip code, insurance type, treatment interest, your screener answers (as selected options only, never free text), the consent text you agreed to, the time and IP address of consent, and the page you came from.
• Calls to tracking numbers. When you call a clinic through a number shown on this site, we log the time, duration and the last four digits of your number. Calls are never recorded.
• Site analytics. Page views with the page path only. No form contents, no text you type, no session recordings.

How we use it

To show your result, to send your information to the clinic or clinics you named, to email you what to expect (three messages, each with an unsubscribe link), to bill the clinic, to detect duplicate or fraudulent submissions, and to keep the service secure.

Who we share it with

• The clinic or clinics you asked us to contact.
• Service providers that deliver email, text messages and phone calls, host our database, process payments from clinics, and monitor errors. They act on our instructions.
• Authorities when required by law.

We do not sell personal information and do not share it with advertisers.

Health information

Covered Mind is not a health care provider and is not a HIPAA covered entity. We deliberately do not collect diagnoses, medication names or clinical notes. Once your information reaches a clinic it is protected by that clinic's privacy practices and, where applicable, HIPAA.

Retention

Contact submissions are kept for 24 months for billing disputes and legal requirements, then deleted. Analytics data is aggregated and does not identify you. You can ask us to delete your submission sooner.

Your rights

You can ask what we hold about you, correct it, or delete it by emailing support@coveredmind.com. Residents of California, Colorado, Connecticut, Virginia, Utah and other states with privacy laws have additional rights, which we honor for everyone. Texting STOP to any message from us ends text messages. Every email has an unsubscribe link.

Cookies

We use no advertising cookies. Analytics runs in memory without a persistent identifier. Security services may set a short-lived cookie to tell people from bots.

Children

This site is for adults. We do not knowingly collect information from anyone under 18.
