What Spravato is and is not approved for
Spravato is approved for treatment-resistant depression and for depression with suicidal thoughts. It is not approved for PTSD, and insurers do not cover it for PTSD alone. Where it comes in is the common situation of PTSD with major depression that has not responded to antidepressants: the depression indication can qualify you. What the evidence says
Trials of esketamine specifically for PTSD are small. Racemic ketamine infusions have shown short-term PTSD symptom reduction in several controlled trials, which is why ketamine clinics, not Spravato centers, are where most PTSD-focused treatment happens.
If you have PTSD and depression
Ask the clinic to document the depressive disorder, the failed antidepressants and the current oral antidepressant. The prior authorization is for depression; the PTSD is context. Trauma-focused psychotherapy continues alongside.
