The limits
Spravato is not approved or covered for OCD. First-line OCD treatment is exposure and response prevention therapy with an SSRI at high dose; TMS is FDA cleared for OCD and is the interventional option with the clearest role. See the coverage hub, the Spravato hub, and the coverage checker.
Where esketamine fits
People with OCD frequently have major depression. When that depression is treatment resistant, Spravato can be authorized for the depression, and mood improvement sometimes makes OCD therapy more workable. There is no good evidence that esketamine reduces obsessions or compulsions directly.
